  • Comprehensive Medical History Evaluation including 25 page detailed Healthy Aging Lifestyle Assessment and Review of your Old Medical Records which you complete and provide to me in advance,
  • Pre-Consultation Evaluation Time-I will spend an hour and a half prior to your consultation reviewing your comprehensive medical history evaluation, reviewing any old medical records which you have submitted, reviewing your current laboratory evaluation and preparing individualized recommendations for your healthy aging program.
  • Two and a Half Hour Private Office Consultation with me.

The Semi-Annual Professional Fee of $995 includes unlimited consultation via e-mail, phone and in-office at periodic intervals and on an as needed basis. This includes a formal phone follow-up consultation at 6 weeks, 3 months and 6 months- which are typically half hour long. The Semi-Annual Professional Fee also includes an hour long either 6 month or 1 year comprehensive consultation in my office which is one hour long. I must reevaluate you in person in my office at least once every year. This is an opportunity for me to reexamine you, review your program and your progress, review your laboratory evaluation and make any necessary changes to your program.

Due to the nature of the medicine which I practice and that many of the medications which I prescribe are controlled substances requiring an additional license (DEA license), I require that the semi-annual professional fee be paid at the beginning of the 6 month period. This assures that you will have the individualized access that you need during that time. I do not want to discourage patients from contacting me by charging them by the e-mail, phone call or office visit. Since I exclusively work as your consultant, I am able to offer you a direct relationship with the pharmacy and lab which results in a 50% savings on the program. Also, you only incur the inital evaluation cost once. The cost of the program is half as much after the second year as the in-office consultation is included in your semi-annual professional fee. What is the cost of lab follow-up?

I offer lab follow-up reevaluations to my patients at my cost which is $300 for a hormone follow-up lab panel and $400 for a comprehensive lab reevaluation. Lab Reevaluation is done through the partner lab at over 50,000 lab draw sites in the US. There is NO third party mark up to my patients for ongoing lab reevaluation.

How often are lab reevaluations done?

There is always an initial lab reevaluation done after 6-8 weeks after having started the program. Lab reevaluations are then scheduled as needed. Typically there is another lab reevaluation at 6 months and then another at one year. After the first year, lab evaluation is typically done twice a year.

What is the cost of medications and supplements?

Every patient’s program is unique in terms of medications and supplements that are prescribed. The program can be customized to fit your budget. To provide some idea, Testosterone Hormone Replacement by injection for men is about $68 for a 6 month supply.

Do you accept insurance?

I am considered an out of network provider since I do not work directly with or for the insurance providers or any third party.

I do provide patients, at their request, with the itemized documentation necessary to submit to their insurance providers for reimbursement. Many of my patients have been successful in obtaining at least partial reimbursement. Also, you may like to use a Health Savings Account (with a high deductible insurance plan) to pay for your medical expenses.

Do you have a referral program?

Yes, I do offer my established patients a Referral Program to express my gratitude for new patient referrals. This means that once you are a patient and have paid your first semi-annual professional fee, you will receive a credit of $150- for each person who you refer and who becomes a patient- towards your next semi-annual professional fee-up to 5 referrals per 6 month period. I have had many patients take advantage of this program and it allows me to build my practice on referrals, which I very much prefer.
